Job Summary:
The Medical Records Clerk is responsible for maintaining accurate, complete, and confidential patient medical records in a busy cardiology practice. This role supports physicians, nurses, and administrative staff by ensuring timely access to patient information, processing medical record requests, and managing electronic health records (EHR) in compliance with HIPAA regulations.
Essential Duties and Responsibilities
· High school diploma or GED required; associate degree preferred.
Previous experience in a medical office or health information management preferred.
· Experience using Electronic Health Record (EHR) systems.
· Knowledge of HIPAA privacy and security regulations.
· Proficiency in Microsoft Office applications.
· Strong organizational and multitasking abilities.

This role supports physicians, nurses, and administrative staff by ensuring timely access to patient information, processing medical record requests, and managing electronic health records (EHR) in compliance with HIPAA regulations.
Essential Duties and Responsibilities
- Maintain, organize, and update electronic and paper patient medical records.
- Scan, index, and upload clinical documents, including cardiology reports, consultation notes, diagnostic test results, and hospital records into the EHR system.
- Retrieve patient records for physicians, nurses, and authorized staff as needed.
- Process requests for medical records from patients, healthcare providers, insurance companies, and other authorized entities in accordance with HIPAA regulations.
- Verify patient demographic and insurance information for accuracy.
- Ensure documentation is complete before filing records.
- Monitor incoming laboratory results, imaging reports, electrocardiograms (EKGs/ECGs), echocardiograms, stress test reports, Holter monitor reports, and cardiac catheterization reports for proper filing.
- Assist with chart preparation for upcoming patient appointments.
- Maintain accurate records of released medical information.
- Respond to telephone calls, faxes, and electronic requests related to medical records.
- Support quality improvement initiatives by ensuring documentation meets regulatory and practice standards.
- Assist with administrative projects and other front-office duties as assigned.
